Intended Audience: White Label Partners

The following steps will show you how to check if you are a USAC contributor for FUSF exemption.

 

 

  1. Go to the FCC Form 499 Filer Database website.
     
  2. Search for your company by name of Filer ID.
     
  3. Click Submit Search

     
  4. Select your company name from the search results.
     
  5. Search for the Yes or No under the Universal Service Fund Contributor section.
